Lip Lines & Perioral Wrinkles in East Meadow, Long Island

Fine vertical lines above the lip. Lipstick bleeding into tiny creases. Deeper lines visible even when your mouth is relaxed.

They may look similar, but they do not necessarily come from the same thing.

At LIPS NY by Rainforest Medical Aesthetics, we look at how your mouth moves, how deeply the lines are etched into the skin, whether volume and support have changed, and the quality of the surrounding skin before deciding what treatment makes sense.

Because the best treatment for a moving line may not be the best treatment for an etched-in line.

What are perioral
lines?

Perioral lines are wrinkles that develop around the mouth, particularly the vertical lines extending outward from the lip border.

You may also hear them called:

lip lines

vertical lip lines

upper-lip wrinkles

or

smoker’s lines

You do not have to smoke to develop them.

Repeated movement of the orbicularis oris, the circular muscle surrounding the mouth, contributes to vertical creasing over time. Genetics, aging, loss of tissue support, skin changes, and cumulative sun exposure can also contribute.

That is why two people with similar-looking lip lines may need very different treatment plans.

First question: do the lines appear only when you move?

Dynamic lip lines

Some lines become most visible when you:

  • purse your lips;
  • drink through a straw;
  • pronounce certain words;
  • whistle;
  • kiss; or
  • tighten the mouth.

These are strongly influenced by muscle movement.

In selected patients, very conservative neuromodulator treatment may reduce excessive folding caused by orbicularis-oris activity.

But weakening this muscle too much can affect normal mouth function.

The orbicularis oris helps you speak, sip, pucker, whistle, and maintain lip seal.

That is why perioral toxin treatment requires considerably more restraint than simply “chasing every line.”

What if I can see the lines even when my mouth is relaxed?

Static or etched-in lip lines are different.

When vertical lines remain visible at rest, muscle activity may still contribute—but the wrinkle has also become structurally established in the skin.

In that situation, relaxing the muscle alone may not erase the line.

Depending on what we see, treatment may involve:

  • dermal filler for selected perioral wrinkles;
  • conservative neuromodulator treatment when repetitive movement remains important;
  • treatment directed toward skin texture and collagen; or
  • a combination approach

Movement. Structure. Skin.

We treat the part that is actually creating the line.

Botox or Xeomin for vertical lip lines

Sometimes the problem is movement.

Small amounts of botulinum toxin can be used around the orbicularis oris to reduce excessive muscular contraction contributing to dynamic vertical lip lines.

At LIPS NY:

Botox or Xeomin — $9.50 per unit

10-unit minimum — $95 minimum treatment

The number of units and injection locations are determined after examining your mouth at rest and during movement.

Botox and Xeomin are approved medications. Treating perioral lip lines is an off-label use.

Perioral wrinkles are not included among the current FDA-approved cosmetic indications for Botox Cosmetic or Xeomin.

Using either product for vertical lip lines is therefore off-label.

That means the use is not one of the cosmetic indications specifically approved in the FDA labeling and should be discussed as part of informed medical decision-making.

Your lips have a job.

The muscles around the mouth help you:

  • speak clearly;
  • sip from a cup;
  • drink through a straw;
  • whistle;
  • pucker;
  • keep liquids in the mouth; and
  • control facial expression.

Too much weakening can temporarily interfere with those functions.

Around the mouth, precision matters more than units.

Filler for vertical lip lines

Sometimes the line needs support—not more muscle relaxation.

If a wrinkle remains visible when your mouth is completely relaxed, the line may be structurally etched into the skin.

In selected patients, hyaluronic-acid filler can provide support beneath these lines.

Juvéderm Volbella XC — $799 per syringe

Volbella XC is FDA-approved for correction of perioral rhytids in adults over age 21.

The goal is not necessarily to create larger lips.

Treating a line and enlarging a lip are not the same thing.

A patient may want improvement in vertical wrinkles without wanting a noticeably fuller mouth. That distinction is part of the treatment plan.

It should not be the objective.

Perioral filler is not simply a matter of adding as much volume as possible.

The amount, depth, product, and location all affect the result.

If your primary concern is a fine etched line, placing unnecessary volume into the lip itself may fail to address what bothers you and may create fullness you never asked for.

We do not use “more filler” as a universal answer to aging around the mouth.

What about the skin itself?

Sometimes the wrinkle is telling us more about the skin than the lip.

Over time, collagen loss, photoaging, and repeated folding can leave the skin around the mouth with fine creasing and textural change.

When skin quality is the dominant issue, injectable treatment alone may not address everything you see.

Rainforest Medical Aesthetics offers skin-focused treatments including fractional radiofrequency microneedling and a 1540 non-ablative fractional laser for fine lines, texture, and collagen remodeling.

Fractional RF Microneedling

Full Face — $499

1540 Non-Ablative Fractional Laser

Full Face — $799

3-session package — $1,999

Whether either treatment is appropriate for your perioral skin depends on your skin type, medical history, degree of wrinkling, tolerance for downtime, and treatment goals.

So which is better: Botox, filler, or resurfacing?

None is universally “better.”

They do different things.

TOXIN

Best considered when repetitive muscular folding is an important contributor. Botox or Xeomin — $9.50/unit 10-unit minimum

FILLER

Best considered when a line needs structural support or volume correction. Juvéderm Volbella XC — $799/syringe

SKIN-FOCUSED TREATMENT

Best considered when texture, collagen loss, and etched skin changes are important contributors. Fractional RF Microneedling — $499 full face 1540 Fractional Laser — $799 full face

COMBINATION TREATMENT

Sometimes the line exists because all three factors are present.

That is when combining treatments may make more sense than asking one product to solve every part of the problem.

Because smoking is not the only cause.

The term smoker’s lines is catchy, but it is not a diagnosis.

You can develop vertical perioral wrinkles without ever smoking.

Repetitive mouth movement, genetics, aging, sun exposure, collagen changes, and tissue-volume changes can all contribute.

At LIPS NY, we prefer “vertical lip lines” or “perioral lines.”

It describes what you see without pretending we know why you have them.

Potentially.

If you want both lip enhancement and improvement in perioral lines, those goals can be evaluated together.

That does not mean the same amount of filler should automatically be placed in both areas.

You may love your lip size but dislike the vertical lines above it.

You may want fuller lips but have almost no perioral wrinkles.

Or you may want a comprehensive rejuvenation of the lips and surrounding mouth.

Same area. Different goals.

The treatment plan should say exactly what we are trying to accomplish.

Sometimes the treatments overlap—but the goals are different.

A lip flip reduces selected orbicularis-oris activity to allow greater upper-lip eversion.

Conservative toxin used for dynamic perioral lines also affects orbicularis-oris activity.

That does not mean every lip flip treats wrinkles or every wrinkle treatment should create a lip flip.

The amount, placement, and desired effect matter.

We start with what you want changed rather than assigning a trendy treatment name first.

What happens at a lip-line consultation?

1. We look at your mouth relaxed.

If the lines are clearly visible without movement, that tells us something about their structural component.

2. We ask you to move.

Purse your lips.

Speak.

Smile.

The lines that suddenly deepen tell us how much muscle activity is contributing.

3. We look at the skin.

Fine crepiness and textural changes may point toward a skin-quality component that filler or toxin alone cannot fully address.

4. We discuss volume.

Loss of lip or perioral support may be part of the picture—but we do not assume every wrinkle means you need larger lips.

5. We build the treatment around the cause.

That may mean toxin.

Filler.

Skin-focused treatment.

A combination.

Or sometimes no treatment at all.

Lip Lines & Perioral Wrinkle FAQs

How much does Botox for lip lines cost at LIPS NY?
Botox or Xeomin is $9.50 per unit with a 10-unit minimum, making the minimum toxin treatment $95.
Juvéderm Volbella XC is $799 per syringe.

Filler is priced per syringe at Rainforest Medical Aesthetics, but the amount and placement used for any treatment are based on your clinical plan.

Not necessarily. Toxin is most relevant to the movement component of a wrinkle. A line that remains deeply etched at rest may need a different or combined approach.
Neither is universally better. Toxin targets muscle activity. Filler provides structural support. Skin-focused treatments address texture and collagen.
Yes. Treatment can be directed toward perioral lines without making lip enlargement the primary goal.
Absolutely. Smoking is only one potential contributor to vertical perioral wrinkling.
No. Perioral lip-line treatment is an off-label use of Botox Cosmetic and Xeomin.

DON’T TREAT EVERY LINE THE SAME WAY.

A moving line is different from an etched line. An etched line is different from lost volume. And lost volume is different from aging skin.

Show us what you see.

We’ll figure out what’s creating it.
